Our EMBRACE Care Manager performs a cognitive assessment and creates a care plan. An initial activity session is scheduled to start the program. Clients then benefit from regularly scheduled meaningful engagement sessions and ongoing monitoring to adjust the plan of care as needed.
Our approach includes activities that bring joy and purpose, along with strategies to support brain health, helping your loved one enjoy a better quality of life now and in the future.
An EMBRACE Care Manager performs a comprehensive assessment to discover the client’s needs and create a personalized
plan of care.
Clients benefit from cognitive and functional activities tailored to the unique needs of each client with the goal to slow disease progression.
The care team monitors the clients' progression throughout the EMBRACE program and adjusts the plan of care as needed.
